SA plans anti-HIV jab roll-out at hundreds of clinics by April –

Subedar says Sahpra has indicated that it could register the medicine as early as October, but she cautions that this is dependent on the regulator receiving all necessary paperwork from Gilead Sciences and the other generics manufacturers. 

“We still need to see the finalised guidance on preparing for the LEN roll-out,” Subedar says. “We need to plan for the roll-out and procurement of the medicine, but we can’t do this until we have a registered product.” 

She adds: “We also need to see the final guidance on who should receive the medicine and how we will monitor for side effects and effectiveness.” 

Registration documents for LEN were submitted to Sahpra by Gilead and the generics manufacturers in June, says Subedar. 

“We know that Sahpra has been working hard to review the data for the medicine, but, of course, we can’t rush the process.”

Registration of medicines is a crucial step in ensuring the safety, efficacy, and quality of drugs available in a country. Without registration, medicines cannot be legally used or sold. Recently, South Africa’s Sahpra took part in the EU-M4All programme, which helped expedite the review process for a new drug. The drug was approved on July 24, paving the way for its registration in the country.

Sahpra’s CEO, Boitumelo Semete-Makokotlela, mentioned that the country now has 90 work days to complete the registration process and start using the new drug. Donors, policymakers, and activists are hoping for a swift registration process, with some aiming for approval as early as October.

One of the key aspects of registering a new medicine is determining its cost. In the case of this new drug, the pricing details are still under wraps. The Global Fund has requested countries to budget a certain amount per patient per year, but the actual selling price is undisclosed. The health department is navigating the complexities of pricing transparency while aiming to secure the drug for those in need.

Groups that are at a higher risk of contracting HIV, such as teen girls, young women, gay and bisexual men, transgender individuals, sex workers, and pregnant or breastfeeding women, will be prioritized for the new drug. The hope is that this new medicine, which only needs to be taken twice a year, will revolutionize HIV prevention efforts in the country.

The introduction of this new drug could be a game-changer in the fight against HIV. With a simplified dosing schedule and potentially higher adherence rates, experts are optimistic about its impact. A modelling study suggests that widespread use of this drug could significantly reduce new HIV infections and bring an end to AIDS as a public health threat by 2032.

Overall, the registration and availability of this new drug represent a significant milestone in South Africa’s efforts to combat HIV/AIDS and improve public health outcomes. It highlights the importance of timely registration processes and access to innovative treatments to address pressing health challenges.

In the year mentioned, there were 73,000 new HIV infections, surpassing the number of HIV-positive individuals who passed away. The Global Fund funding for LEN falls short of covering the two to four million people in need each year, only reaching between 9% and 19% of this group. Additionally, generic versions of the medication will not be available until 2027, making it highly unlikely to achieve the goal of providing treatment to all those in need.

According to Yogan Pillay, who oversees HIV and TB service delivery at the Gates Foundation, the current focus is on ensuring there are no delays in the production of generic LEN. The foundation is collaborating with one of the six companies that have obtained voluntary licenses to help them expedite the production process and bring the generic medication to market by early 2027. In the event that the company produces more lenacapavir than is needed, the foundation will cover the production costs.

Unitaid, an organization hosted by the World Health Organization, is also working with some of the licensed companies to accelerate the production of generics at the lowest possible cost. Both the Gates Foundation and Unitaid will soon announce the companies they are partnering with to achieve this goal.

South Africa is expected to be the leading market for LEN, followed by Zambia, Uganda, and Nigeria. Mitchell Warren, executive director of Avac, highlights that while generic manufacturers aim to make a profit, competition among the companies granted licenses is likely to drive down prices and eventually make LEN available at a more affordable cost, similar to the daily HIV prevention pill.
