Living organ donors need better long-term medical support

The case of my sister Robin and my own experience as a kidney donor highlight the long-term health risks that living organ donors face. While the act of donation is often framed as a selfless and heroic act, the reality is that donors can experience significant health complications that are not adequately addressed or monitored.

For Robin, her decision to donate a kidney to our brother ultimately led to a series of health issues that culminated in her untimely death from breast cancer at the age of 51. Her struggles with managing her health post-donation were exacerbated by the lack of guidance and support for donors facing health challenges later in life. The adverse events she experienced, such as hydronephrosis and renal impairment, are not uncommon among living donors, yet there is a glaring lack of data on the long-term outcomes and risks associated with donation.

Similarly, my own journey as a kidney donor has been fraught with challenges. From complications during pregnancy to injuries sustained during medical procedures, I have faced numerous health issues that have been directly linked to my decision to donate a kidney. The lack of comprehensive data on donor welfare, coupled with the systemic focus on increasing organ supply at the expense of donor safety, has left donors like myself vulnerable and unsupported.

The current landscape of living organ donation is characterized by a lack of accountability and transparency when it comes to tracking donor outcomes. Despite efforts to incentivize donation through tax breaks and insurance protections, the fundamental issue of ensuring donor safety and well-being remains largely unaddressed. The focus on increasing donations without prioritizing donor health and safety perpetuates a system that places donors at risk and fails to provide adequate support for those who selflessly give the gift of life.

As we continue to grapple with the complexities of living organ donation, it is crucial that we prioritize the long-term health and well-being of donors. Comprehensive data collection, robust follow-up care, and proactive measures to address donor health needs are essential components of a system that truly values and protects those who make the ultimate sacrifice to save the lives of others. It is time to shift the paradigm and ensure that living organ donors receive the care and support they deserve, both during and after the donation process. Access to Medicare benefits is crucial for individuals who have undergone living organ donation, as commercial insurance and state Medicaid plans can be unpredictable and inconsistent. By adding a “prior donor” modifier to CMS-1500 standardized billing forms used by health care providers, we can ensure the collection of high-quality health data. This step is essential in overcoming research limitations and addressing the ongoing resistance of UNOS/OPTN to document the long-term health impacts of living donations.

The Health Resources and Services Administration (HRSA) is taking steps to modernize the Organ Procurement and Transplantation Network (OPTN), with a new board of directors set to assume leadership soon. However, the impact of these reforms, along with recent staffing and budget cuts at the Department of Health and Human Services (HHS), on the care and support of living donors remains uncertain.

Robin, a living kidney donor, felt a sense of pride in her decision to donate, even though her kidney did not ultimately help her brother. Similarly, my own kidney donation did not save our brother, who tragically passed away after years of dialysis and failed transplant attempts. Throughout his journey, he maintained his emotional depth and intellectual integrity. He would have been pleased to know that our stories as donors may inspire a future where donor well-being is a top priority, and where their life-saving gifts are met with the support and care they deserve.

Jane Zill, a Licensed Independent Clinical Social Worker and living kidney donor who has served on a national committee related to living organ donation, shares her insights and experiences in navigating the complexities of living donation. Her expertise and advocacy highlight the importance of prioritizing donor well-being and ensuring that donors receive the support they need throughout the donation process.

In conclusion, the integration of a “prior donor” modifier on billing forms, coupled with ongoing efforts to modernize the OPTN, holds promise for improving the care and support of living donors. As we move forward in advancing organ donation practices, it is essential to prioritize the well-being of donors and ensure that they have access to the necessary resources, including Medicare benefits, to support their health and recovery post-donation.
