Should Hepatitis B-positive mothers breastfeed?

Breastfeeding for Hepatitis B-Positive Mothers: What You Need to Know

Breastfeeding is a crucial aspect of infant health, providing numerous benefits that far outweigh any potential risks. Even for mothers who are hepatitis B positive, breastfeeding is still recommended, with certain precautions in place to ensure the safety of the baby.

First and foremost, the World Health Organization (WHO) guidelines emphasize the importance of administering the hepatitis B vaccine to all infants as soon as possible after birth, preferably within 24 hours. This initial dose must be followed by two or three more doses to complete the primary series, effectively preventing mother-to-child transmission of the virus.

Additionally, pregnant women who test positive for hepatitis B infection are advised to receive Tenofovir prophylaxis from the 28th week of pregnancy until birth. This antiviral agent targets viral replication, reducing the risk of transmission to the baby during delivery.

For infants born to hepatitis B-positive mothers, post-exposure measures such as the simultaneous administration of the hepatitis B vaccine and hepatitis B immunoglobulin (HBIG) soon after birth can further minimize the risk of transmission through breastmilk. This approach aims to stimulate the baby’s immune system to produce its own immunity against the virus while neutralizing any circulating viral particles.

Studies have shown that early initiation of post-exposure prophylaxis, ideally within 12 to 24 hours after birth, is highly effective in preventing hepatitis B virus transmission. It is essential for all mothers to undergo hepatitis B testing during pregnancy to determine the appropriate prophylactic measures.

With the availability of the pentavalent vaccine in the expanded immunization program, the number of hepatitis B-positive mothers is declining, offering hope for the eventual elimination of hepatitis B transmission to infants. In summary, here are the key points to remember:

– Hepatitis B-positive mothers with high viral DNA levels should receive Tenofovir from 24 weeks of pregnancy.
– Infants should receive the hepatitis B vaccine or HBIG within 12 hours of birth, with subsequent doses according to the recommended schedule.
– Post-vaccination testing for hepatitis B antibodies should be conducted at nine to 18 months of age.

By following these guidelines and recommendations, mothers can breastfeed their babies safely, ensuring optimal health outcomes for both mother and child.
