US: Lifesaving Programs Remain Suspended Despite Waivers

Millions at Risk as Vital Programs Shut Down

The recent suspension of $44 billion in foreign aid by the US State Department has led to the shutdown of numerous vital programs worldwide, putting the health and well-being of millions of individuals at risk. Despite waivers issued by the State Department, many crucial programs focused on human rights, healthcare, and democracy promotion remain on hold, causing widespread disruptions and confusion.

On January 20, 2025, President Donald Trump signed an executive order suspending foreign assistance administered by the US State Department, USAID, and PEPFAR. This abrupt decision has had far-reaching consequences, with stop-work orders being sent to implementing partners and contractors globally. Programs addressing infectious diseases, human rights abuses, and HIV prevention have been severely impacted, jeopardizing the lives of countless individuals.

One of the most critically affected programs is PEPFAR, which provides essential HIV prevention and treatment services to millions of people. The suspension of funding has disrupted crucial services such as prevention of mother-to-child transmission of HIV, putting pregnant individuals and newborns at increased risk. Without access to lifesaving medicines and care, the health of vulnerable populations is at stake.

While the State Department has issued waivers allowing for the resumption of certain humanitarian assistance programs, the lack of clear guidance and staffing disruptions have hindered the restart of operations. Conflicting information and the closure of USAID have further complicated the situation, leaving many programs in limbo.

The impact of these suspensions is already being felt, with a significant increase in HIV infections reported since the halt of preventative programming. The inability to access antiretroviral treatments and other essential medicines poses a grave threat to those relying on PEPFAR services. Without immediate intervention, the health outcomes of millions of individuals could be compromised.

Human Rights Watch has called on Secretary of State Marco Rubio to take urgent action to restore suspended services and ensure the swift resumption of vital programs. The rehiring of federal employees and contractors who oversee these projects is essential to mitigate the ongoing crisis and prevent further harm to those in need.

It is imperative that the State Department prioritizes the well-being of vulnerable populations and acts swiftly to address the repercussions of the foreign aid suspension. The future of millions of individuals hangs in the balance, and decisive action is needed to prevent further harm and ensure the continuity of lifesaving programs.
